Full mailing / physical address of the clinic.

Clinic landline or general contact number.
Clinic email:
Must be a valid email address used for general clinic correspondence.
Doctor in charge:
Name of the doctor in charge of the clinic.
Doctor in charge mmc no:
Malaysian Medical Council (MMC) registration number.
Business operation hours:
Free text describing operating days and hours.
