Name the enterprise clinic you expect to be a child of (required for Enterprise-managed). The pharmacy confirms the actual parent on approval.
Clinic name:
The primary clinic identifier as it should appear on records.
Clinic address:
Full mailing / physical address of the clinic.
Clinic contact no:
Clinic landline or general contact number.
Clinic email:
Must be a valid email address used for general clinic correspondence.
Doctor in charge:
Name of the doctor in charge of the clinic.
Doctor in charge mmc no:
Malaysian Medical Council (MMC) registration number.
Business operation hours:
Free text describing operating days and hours.
